Use this form to seek leave to appeal from a decision of the Victoria Civil and Administrative Tribunal under s 148 of the Victorian Civil & Administrative Tribunal Act 1998.

Note: The Trial Division hears appeals against decisions made by a Member, Senior Member or a Deputy President of VCAT. The Court of Appeal hears appeals against decisions made by the President or a Vice President of VCAT.
