This template may be used where information is needed about how a will came to be prepared. This evidence is usually required when the will was prepared by a family member, friend or other person who was not a lawyer.
The template asks for information about:
- how the will was prepared and signed;
- the instructions and wishes expressed by the deceased;
- the involvement of the person who prepared the will and any other person involved in its preparation; and
- the circumstances that may assist the Court to understand how the contents of the will were decided.
The template is designed to assist the Court by providing evidence about what the person who prepared the will personally saw, heard and experienced.
