These affidavit templates may be used where information is needed about whether a person was the deceased's domestic partner.

Use these templates if you are applying for letters of administration and say that you were the deceased's domestic partner.

These templates help provide information about the relationship between the deceased and the person applying for the grant.

The Court may need information about matters such as:

  • how long the relationship existed;
  • whether the parties lived together;
  • how they shared their lives;
  • financial arrangements;
  • household arrangements; and
  • how they were known to family, friends and the community.

This affidavit should be completed by the person who says they were the domestic partner of the deceased.  It should explain the relationship from when it began until the deceased’s death. 

This affidavit should be completed by a person who knew the deceased and the person claiming to be the deceased’s domestic partner.
