This form is to be used where the law firm acting for a party to an existing proceeding ceases to act for that party.

This form must include the last known address of that party and the last known email address of that party.

If the proceeding is not a contested matter, use the form “Notice of ceasing to act – Probate Office” below.

If the proceeding is a contested matter, use the form “Notice of ceasing to act – contested” below.
