Form 64C - List of persons served

The purpose of Form 64C is to confirm that an application or appeal has been served, and to provide the Court with contact information for the person served.

The Court rules require a party who has filed and served an application or an appeal to also file a Form 64C within 7 days after service. This applies to service of an application for leave to appeal, a notice of appeal, a cross-application for leave to appeal, a notice of cross-appeal and an application other than for leave to appeal.
