Affidavit of service

Affidavit template to be used when making an affidavit of service for a proceeding in the Trial Division of the Supreme Court.

Notice of change of address for service

This form is to be used where a party to an existing proceeding changes their address for service.

Notice of change of solicitor

This form is to be used where a party to an existing proceeding who is legally represented appoints a different law firm to represent them.

Form 48A – Notice of Trial

This form is to be used in all lists in the Trial Division excluding the lists listed in order 48.1(2) of the the Supreme Court (General Civil Procedure) Rules 2015 and proceedings which are directed to file a form 48B.

Form 48B – Notice of Trial

This form is to be used in the Judicial Review and Appeals List and any other proceeding where the order specified that the Notice of Trial shall be in Form 48B.
