This form is to be used to identify an exhibit referred to within an affidavit.

This form is to be used where a party to an existing proceeding who is not legally represented appoints a solicitor to represent them.

This form is to be used where a party to an existing proceeding changes their address for service or where a different solicitor within a firm now acts for that party.

This form is to be used where a party to an existing proceeding who is legally represented appoints a different law firm to represent them.

This form is to be used where the law firm acting for a party to an existing proceeding ceases to act for that party.

These affidavits are to be filed in support of an application for letters of administration by: a person who is not entitled by law to share in the estate, or a person who is not a beneficiary under a will.

Form 42AA Subpoena for Production to the Prothonotary is used when issuing a subpoena for production of documents or things to the Prothonotary.

Form 42A Subpoena to Attend to Give Evidence is used to compel a person to attend to give evidence at the trial of a proceeding.

Form 42B Subpoena to Produce is used to compel a person to produce documents at the trial of a proceeding.
